Things you need to know to complete the form:

1. Complete the appropriate request form.

2. Anyone can apply for the deceased.

3. Must be delivered to a place of business (not a private residence). Businesses must sign the form for delivery. Please be sure to let us know in advance if you have it sent to us at 3090 Davison Rd. Lapeer, MI 48446.

Things you will need to complete the form:

1. Death Certificate.

2. Discharge Papers - DD214.

3. Social Security Number.
